Woodbank Surgery regret to inform that Dr Shawn Sacks will be leaving us on Wednesday 20th February 2019


All the staff at Woodbank Surgery wish Dr Sacks well for the future and thank all patients for their continuous support.

As from the 1st August 2018 Woodbank Surgery will now be 


Closing at 18:30 on Mondays, Wednesdays, Thursdays and Fridays 


We are still open to fulfill our contractual obligations with Extended Hours on a Tuesday evening from 18:30 to 20:00

New Patient Registrationregister_now

If you live in our practice area and wish to register as a patient at our surgery then we need certain information from you and each member of your family so that we are able to trace your previous medical records. We advise you here on how you may register as there are several methods of doing so.

 

  • You may call into the surgery bringing with you your NHS medical cards from your previous NHS doctor.
  • You may call into the surgery without bringing with you your NHS medical cards from your previous NHS doctor. This may arise if you cannot find your cards or if you do not have any e.g. if you have recently arrived in the country. This is less preferable since it may take longer to trace your medical records and you will have to fill in more paperwork, so please find your medical cards if you can. 

Please note that we cannot guarantee that we shall accept you as a patient since, for example, it may be that your address is not in our catchment area.

New Patient Check

When you first register we will not have any information about your health from your previous records. You will be encouraged to book for an initial check-up when you register.

If You Change Your Contact Details

If you change your name, address, or telephone number (including mobile number) please tell us. If we do not have your current details this will cause delay or maybe even make it impossible for us to contact you. You may inform us by post or by calling in to fill in a form at reception.

 

Make sure that you tell us which members of your household have moved with you - we cannot assume that all have moved with you since sometimes families split up or children move away to college etc. 

 

Please tell us your telephone number even if you are ex-directory. We will not disclose it to anyone, but it is sometimes important that we are able to contact you quickly. 

 

Note that if your new address is outside our practice area we may not be able to keep you as a patient at this practice. In that case we shall contact you to let you know that you need to find a practice in your new locality.
